For your convenience, we've compiled a list of commonly used recorder forms.

The Keokuk County Recorders office is responsible for recording all real estate documents; issuing boat, snow and ATV registrations and titles; issue hunting and fishing licenses; filing birth, death and marriage records; and issuing certified copies of vital records in Keokuk County.

Declaration of Value

Pursuant to Iowa Code section 428A.7 as amended by 2026 Iowa Acts, Senate File 2472, the Iowa Department of Revenue has published a supplemental schedule to the Declaration of Value (Real Estate Transfer - Declaration of Value Sales Condition Reporting) to allow buyers, sellers, or their agent to report sales conditions.

Redaction Forms for Law Enforcement

Section 331.604 subsection 3(f) provides for the redaction of certain names from electronic documents displayed for public access through an internet site for the protection of current and former law enforcement professionals. This form is required for the processing of any redaction request under this section.
